Autonomous haul truck collided with a manned water cart

On 11 August 2015, an autonomous haul truck collided with a manned water cart at an Australian open-pit mine, as recorded by WorkSafe Western Australia.

The incident occurred during night-shift operations while autonomous trucks were hauling waste material. A control room operator instructed an autonomous haul truck to turn right at an intersection and complete a loop to position itself under an excavator.

Although this intersection existed digitally in the autonomous control system, it was not physically marked or signposted on the ground, leaving manned vehicles unaware of the autonomous truck’s programmed route.

As the autonomous truck began its right turn, a water cart approached from the opposite direction. The water cart operator, unaware of the truck’s intended path, attempted evasive action after recognising the conflict too late.

The two vehicles collided, causing significant damage to the autonomous truck and resulting in minor injuries to the water cart operator. The event exposed gaps in interaction management between autonomous and manned fleets during mixed-mode operations.

This collision reveals important functional safety lessons:

  • Autonomous system safety must not rely solely on OEM perception or navigation logic; independent validation is essential.
  • Interaction areas between autonomous and manned vehicles require engineered segregation controls, not just procedural separation or operator judgment.
  • Unexpected autonomous vehicle behaviour highlights inadequate diagnostic coverage and weak failover design, requiring more robust fault detection and safe-state responses.
  • Site operators must verify autonomous logic, braking behaviour, and response times, including performance under simulated fault conditions.
  • Human–system interaction issues, such as the water cart operator’s situational awareness, must be examined through CHAZOP/HAZID processes to ensure safe coordination.
